Hip replacement plan कर रहे patients के मन में सबसे common सवाल होता है — Hip Replacement के लिए कौन सा Implant Best है?

एक experienced Hip Replacement Surgeon in India के तौर पर, मैं अक्सर patients को समझाता हूं कि implant की choice मुख्य रूप से दो चीजों पर depend करती है। पहली है bearing surface, यानी cup और head किस material के बने हैं, और दूसरी है कि stem cemented है या uncemented।

इस article में हम इन दोनों factors को detail में समझेंगे और जानेंगे कि Hip Replacement के लिए कौन सा implant combination बेहतर माना जाता है।

Hip Replacement में कौन-कौन सी Bearing Surfaces होती हैं?

Hip Replacement में मुख्य रूप से तीन तरह की bearing surfaces का use किया जाता है:

• Metal-on-Poly — इसमें cup की lining poly यानी highly cross-linked polyethylene की होती है, जबकि femoral head metal का होता है।
• Ceramic-on-Poly — इसमें cup की lining poly की होती है, लेकिन femoral head ceramic का होता है।
• Ceramic-on-Ceramic — इसमें cup की lining और femoral head दोनों ceramic के होते हैं।

इन अलग-अलग bearing surfaces को develop करने का मुख्य उद्देश्य implant में wear and tear को कम करना है। Implant जितना कम घिसेगा, उसके लंबे समय तक चलने की संभावना उतनी ही बेहतर होगी। इसी वजह से Hip Replacement में अलग-अलग hard और durable materials का use किया जाता है।

Hip Replacement में सबसे Best Combination कौन सा है?

अगर Australian Joint Replacement Registry जैसे international data को देखें, तो आज के समय में Ceramic-on-Poly combination काफी commonly used है।

इसमें cup highly cross-linked poly का होता है और femoral head ceramic का। इसे आप एक “hard and soft combination” की तरह समझ सकते हैं — एक तरफ ceramic hard होता है, जबकि दूसरी तरफ poly comparatively soft material होता है।

Available data के अनुसार, Metal-on-Poly implants की life Ceramic-on-Poly की तुलना में थोड़ी कम हो सकती है।

हालांकि, अगर आपका budget limited है, तो Metal-on-Poly भी एक reasonable option हो सकता है। Ceramic head और Metal head की price में लगभग ₹20,000–₹25,000 तक का difference हो सकता है।

लेकिन अगर price आपके लिए major concern नहीं है, तो Ceramic-on-Poly एक बेहतर combination माना जाता है।

Ceramic-on-Ceramic Implant के बारे में क्या ध्यान रखें?

कई surgeons Ceramic-on-Ceramic combination का भी use करते हैं। इसमें cup और femoral head दोनों ceramic के होते हैं, यानी दोनों surfaces hard होती हैं।

लेकिन इस combination में दो important concerns हो सकते हैं:

1. Brittleness

Ceramic एक hard material है, लेकिन इसमें fracture होने का risk हो सकता है। इसलिए implant selection करते समय इस factor को भी ध्यान में रखा जाता है।

2. Squeaking Sound

कुछ patients को Hip Replacement के बाद implant से squeaking या आवाज आने की शिकायत हो सकती है। हालांकि यह हर patient में नहीं होता, लेकिन कुछ cases में यह discomfort का कारण बन सकता है।

इसी वजह से worldwide level पर Ceramic-on-Poly combination को भी काफी prefer किया जाता है।

Cemented Stem या Uncemented Stem — कौन सा बेहतर है?

Bearing surface decide करने के बाद Hip Replacement में दूसरा important decision होता है कि stem cemented होना चाहिए या uncemented।

दोनों options के अपने फायदे और limitations होते हैं।

Uncemented Stem में cement का use नहीं किया जाता। Stem directly bone के contact में रहता है और इसकी surface पर highly porous coating या HA coating हो सकती है। इससे osseointegration में मदद मिलती है, यानी समय के साथ bone implant के साथ naturally integrate हो सकती है।

लेकिन Uncemented Stem का result काफी हद तक patient की bone quality पर depend करता है।

अगर patient को Rheumatoid Arthritis, Ankylosing Spondylitis, Osteoporosis या किसी दूसरी condition की वजह से bone quality कमजोर है, तो ऐसे cases में Cemented Stem की जरूरत पड़ सकती है।

Weak bone में Uncemented Stem के loose होने का risk बढ़ सकता है। इसलिए केवल implant के नाम या brand के आधार पर decision नहीं लेना चाहिए।

एक Simple Rule याद रखें

• Bone quality अच्छी है → Uncemented Stem suitable हो सकता है
• Bone quality कमजोर है → Cemented Stem consider किया जा सकता है

लेकिन यह कोई fixed rule नहीं है। Final decision patient की age, bone quality, medical condition और surgeon की clinical assessment के आधार पर लिया जाना चाहिए।

Hip Replacement Implant के Brands कौन से हैं?

Hip Replacement में use होने वाले कई implants international companies द्वारा manufacture किए जाते हैं। इस field की बड़ी companies में Johnson & Johnson, Zimmer और Stryker जैसे names शामिल हैं।

मैं अपनी practice में Johnson & Johnson के implants को ज्यादा prefer करता हूं। हालांकि, final implant selection हमेशा अपने surgeon के साथ discuss करके ही करना चाहिए।

जिस implant system के साथ surgeon experienced और comfortable हो, वह भी implant selection में एक important factor होता है।

क्या सबसे महंगा Implant ही सबसे Best होता है?

यह जरूरी नहीं है कि सबसे महंगा implant ही हर patient के लिए सबसे best हो।

Hip Replacement में implant selection करते समय केवल price नहीं, बल्कि patient की age, bone quality, activity level, medical condition और long-term requirements को भी ध्यान में रखना जरूरी है।

एक implant जो किसी young और active patient के लिए suitable हो सकता है, जरूरी नहीं कि वही implant हर दूसरे patient के लिए भी सही हो।

इसलिए implant को केवल brand या price के आधार पर choose करने के बजाय अपने orthopedic surgeon से proper evaluation करवाना ज्यादा important है।

Hip Replacement Implant को लेकर क्या याद रखें?

Hip Replacement implant को लेकर confusion होना बिल्कुल normal है। लेकिन कुछ important बातें आपको जरूर ध्यान रखनी चाहिए।

• Bearing surface के लिए Ceramic-on-Poly एक commonly preferred combination है।
• Metal-on-Poly भी कुछ patients के लिए एक reasonable और cost-effective option हो सकता है।
• Ceramic-on-Ceramic में brittleness और squeaking sound जैसे concerns हो सकते हैं।
• Stem का selection आपकी bone quality पर काफी depend करता है।
• अच्छी bone quality में Uncemented Stem suitable हो सकता है।
• कमजोर bone quality में Cemented Stem consider किया जा सकता है।
• Implant का final selection patient की individual condition और surgeon की clinical assessment के आधार पर होना चाहिए।
